Supporting the Need

1. Affidavit of Lack of Broadband Availability
You will be asked to provide documentation proving that there is a lack of high-speed access in your territory. Personalize this form with your cooperative specifics and distribute among your membership living in affected areas.
2. Residential Letter of Support
The residents of your community also have an important role in supporting the project. Distribute this form to them for personalizing and submitting on your behalf.
3. Authorized Representative Resolution
You finally have all of your cooperative affidavits returned, reviewed, and confirmed. Now, it’s time to record the support of your board representative(s).

Applying for Funding

4. Grant Application Cover Sheet
Update this document for each specific grant or funding request that your cooperative is submitting. It serves as a checklist for the documents typically required for your application and provides a high-level snapshot of your organization’s needs.
5. Funding Request Resolution
This resolution confirms the partnership and financial commitment of your cooperative, along with Conexon/Conexon Connect. It will be submitted to the funding agency or grantor with your application.
6. Legal Opinion Letter
This template serves as a confirmation of support from your cooperative’s legal counsel and should be completed and endorsed by those professionals.
7. Letter of Commitment
This letter serves as legal confirmation of your cooperative’s intent and ability to participate in the ensuing broadband project. It should be completed by your cooperative’s legal counsel.
8. Net Neutrality Statement
This letter is completed and signed by your cooperative’s CEO. It confirms that all members will receive equal access to fiber services upon completion of the project.

9. Letter of Support v1
These templates are completed by the organization(s) (i.e. Conexon/Conexon Connect, county government) endorsing and validating the need for your broadband project. Please utilize the template that is most applicable to your project.

11. Local Match Fund Commitment Letter
Local partners, such as government entities, should complete this letter with the specifics of their financial commitment to the project. It will be included in your application for funding.
12. RDOF Letter of Credit
To satisfy the requirements of the Rural Digital Opportunity Fund program, a letter of confirmation is required by creditors assisting with the funding of your project. Please have this letter personalized and signed by those participating entities.
13. Bank Letter of Credit
This letter attests that your financial lending institution supports the broadband project.
